Wraparound Facilitator - New wraparound facilitators will join an enthusiastic and supportive team to provide wraparound facilitation for eligible children and youth with significant challenges in the Trillium Behavioral Health (TBH) Wraparound program. Wraparound Facilitators will facilitate Child and Family Wraparound Teams to develop Plans of Care; work with TBH Care Coordinators to arrange needed treatment; coordinate and collaborate with system of care/community partners; and, perform other duties as assigned. Extensive training will be provided from experts in the field. A Bachelor’s degree is preferred or a combination of three years relevant experience.

Family Service Coordinator - Family Service Coordinators support families of children with disabilities, helping find resources, communicating effectively with community resource partners, schools and other service systems, and supporting clients to problem-solve barriers. Spanish speaking individuals strongly desired.

Youth Navigator - The Youth Navigator provides support necessary to follow through on a youth’s individualized service plan and designs services to facilitate positive youth development, engagement in education, and prevent homelessness.
