Wraparound Facilitation is a long-term, team-based planning process that helps youth and families create a coordinated plan of care that fits their unique needs and goals. Each family works with a Wraparound Facilitator and a Wraparound Partner who provide ongoing guidance and support every step of the way.
Our team members:
Support Lane County youth (ages 0–20) with significant behavioral and emotional needs
Take time to get to know each family’s story, strengths, and priorities
Build a supportive team around the youth—including professionals and natural supports—while leading regular meetings and connecting families to helpful resources
