We support Lane County families raising youth with disabilities and complex needs through five connected programs that focus on education, planning, and peer support.
How We Support Families
We help families understand, access, and coordinate services and supports to meet basic needs, including food, housing, medical care, respite, and job placement.
We provide encouragement, emotional support, and continuity to reduce isolation and help families feel more in control of their lives.
We arrange and attend (if necessary) school, medical, mental health, and other important meetings with families to ensure their voice is heard and promote cost-saving collaboration across providers.
We ensure youth and family needs are met in a timely manner, without duplication and confusion, and before more expensive services are required.
Support for school concerns, IEP/504 plans, and special education questions to promote school success.
Intensive, family-driven planning and monthly team meetings for youth with significant behavioral or emotional needs.
